From 2017, the ICD 10 coding guidlines has allowed to code Hypertension and Heart disease together. Earlier we have to look for any causal relationship between Hypertension and heart disease, but with updated guidelines in 2017, you don’t have to look for any causal relationship. Unless and until the physician documents that hypertension and heart diseases are unrelated, you can assume a causal relationship between them. Hence, going forward don’t look for “hypertensive” or “due to” phrases to find a relationship between them. Just assume and code the combination code once you find both hypertension and heart disease documented in the report. If provider documents clearly that they are not related to each other, then only we have to code them separately. The simple rule here we have to use a combination code if there is documentation of hypertension and Chronic Renal Disease anywhere in the medical report.
