The Health Department is always available to respond to public health emergencies. You can call us anytime at (914) 813-5000. We respond to public health concerns such as potential rabies exposure, foodborne illnesses, sewage, chemical or oil spills, public drinking water and bathing beach water quality and preventing the spread of infectious diseases. After hours, we will return those calls that are public health emergencies. Other calls will be responded to the next business day.

License Application Status

Find the status of your license application by checking our online license verifications; your name will be added immediately when your license number is issued.

We cannot provide the status of a licensure application by phone. After you have submitted all your documentation, please allow 6 weeks before submitting a Contact Us Form to request a status update. Contacting us earlier may extend processing times for applications. During busy times, this time frame may increase. We thank you for your patience and cooperation.

Professional Misconduct and Discipline

Professional misconduct by licensed physicians, physician assistants, and specialist assistants is investigated and prosecuted by the Office of Professional Medical Conduct in the New York State Department of Health. Complaints of misconduct in these three professions should be made to the New York State Department of Health, Office of Professional Medical Conduct , Riverview Center, 150 Broadway, Suite 355, Albany, NY 12204-2719.
